📋 THE PROMPT:

Create a customer feedback survey for [PURPOSE].

Survey goal:

  • What I want to learn: [SPECIFIC INSIGHT]

  • Target respondents: [CUSTOMER SEGMENT]

  • Decision it will inform: [PRODUCT/FEATURE/STRATEGY]

  • Survey length: [5/10/15 QUESTIONS]

  • Incentive: [GIFT CARD/DISCOUNT/ENTRY TO WIN]

Generate a survey with:

Opening Section

  • Thank you message

  • Time estimate (be accurate)

  • How data will be used

  • Privacy assurance

Demographic Questions (2-3 questions)

  • Role/industry

  • Company size

  • How long they've been a customer

  • Use case category

Rating Questions (3-4 questions)

  • Overall satisfaction (1-10 scale)

  • Feature-specific ratings

  • Likelihood to recommend (NPS)

  • Ease of use

Open-Ended Questions (2-3 questions)

  • "What's your favorite thing about [product]?"

  • "What's the most frustrating part?"

  • "What would make this a must-have for you?"

  • "What almost stopped you from buying?"

Multiple Choice (2-3 questions)

  • How often do you use [feature]?

  • Which alternatives did you consider?

  • What's your primary use case?

Closing

  • Anything else you'd like to share?

  • Can we follow up with you? (opt-in)

  • Thank you + next steps

Survey best practices:

  • Keep questions neutral (no leading language)

  • One question per question (don't double-barrel)

  • Offer "Other" options for multiple choice

  • Make most questions optional except key ones

  • Show progress bar

🎯 HOW TO USE IT:

  1. Define exactly what you need to learn

  2. Generate the survey

  3. Test with 2-3 people first

  4. Send to targeted segment

  5. Analyze patterns, not individual responses

💡 PRO TIP: Short surveys get more completions. If you need 20 questions, break it into two surveys. People will answer 10 questions twice before they'll answer 20 once.
